I don't count Fossil Roo or Desert Palace because you can't go back to them once you leave anyway. The list includes the Airship Observatory in Dali, Conde Petie and the Mountain Path, the Iifa Tree itself, Pinnacle Rocks, Oelivert, and Esto Gaza and Gulug Volcano. The Iifa Tree roots have come up from the ground on the fourth disc, and block you out of certain places permanently. You can still get the Mayor's Key at this point, but there won't be anyone to give your coffee to. This is why you can only complete the coffee sidequest during the card tournament: that's the only time that Mayor Kapu won't be in his house until the fourth disc (at which point he, too, is presumably dead).
